What changed in 2026
The federal government moved cannabis from Schedule I to Schedule III in early 2026 — a major policy shift we covered in detail in our breakdown of Oregon's medical cannabis program after Schedule III. Federal rescheduling does not replace your OMMP card. Qualifying conditions in Oregon
To qualify for an OMMP card, you need to be diagnosed with a "debilitating medical condition" recognized by the Oregon Health Authority. The list includes:
- Cancer, glaucoma, HIV/AIDS, severe pain, severe nausea, seizures, persistent muscle spasms (including from MS), cachexia, Alzheimer's, post-traumatic stress disorder (PTSD)
- Other conditions a licensed physician believes will benefit from medical cannabis

How to apply
- Get your medical records. Request records from your primary care physician documenting your qualifying condition. The clinic doctor needs to see this before signing the Attending Physician's Statement.
- Book a clinic appointment. Use any of the verified providers above. Telehealth is fastest if you can't get to a Eugene office.
- The doctor visit. A licensed Oregon MD, DO, ND, or NP reviews your records and your condition. If you qualify, they sign the Attending Physician's Statement.
- Submit the state application. Either you or the clinic submits to the Oregon Health Authority via the OMMP online registry. Pay the state fee at this point.
